Position Summary

The Physical Therapist (PT) is responsible for evaluating and treating residents with physical impairments, disabilities, or movement dysfunction in a skilled nursing facility. The PT develops individualized treatment plans to help residents improve mobility, reduce pain, and restore function, promoting the highest level of independence and quality of life.

Essential Duties and Responsibilities

  • Evaluate residents upon admission or referral to determine their functional mobility and physical therapy needs.
  • Develop, implement, and revise individualized care plans and treatment goals based on clinical findings.
  • Deliver evidence-based physical therapy interventions including therapeutic exercise, gait training, balance activities, and use of assistive devices.
  • Collaborate with interdisciplinary team members including physicians, nurses, occupational and speech therapists, and social workers to optimize outcomes.
  • Supervise Physical Therapist Assistants (PTAs) and ensure accurate implementation of therapy plans.
  • Educate residents, families, and staff on exercises, safety techniques, and adaptive strategies.
  • Maintain accurate and timely documentation in accordance with facility policies, regulatory standards, and reimbursement requirements.
  • Participate in care plan meetings and contribute to discharge planning.
  • Comply with infection control protocols and all facility, state, and federal regulations.

Knowledge, Skills and Abilities

  • Strong clinical reasoning, communication, and organizational skills.
  • Knowledge of Medicare and documentation requirements in SNF settings 

Education & Experience

  • Graduate of an accredited Physical Therapy program.
  • Current and valid Physical Therapist.
  • CPR/BLS certification (required).
  • Experience in geriatric or skilled nursing settings preferred.
