Licensed Vocational Nurse (LVN) - CA

Position Summary

The LVN provides compassionate, high-quality nursing care to residents in a skilled nursing facility under the supervision of a Registered Nurse (RN) or physician. This role ensures residents receive appropriate clinical care and support in accordance with their individualized care plans, facility policies, and state and federal regulations.

Essential Duties and Responsibilities

  • Deliver safe, effective, and individualized nursing care to residents.
  • Administer medications and treatments as prescribed and document accurately.
  • Monitor residents' conditions and report changes to the RN, physician, or appropriate clinical staff.
  • Assist with admissions, discharges, and transfers of residents, including completion of required documentation.
  • Perform wound care, catheter care, and other nursing procedures within the LVN scope of practice.
  • Participate in care planning and interdisciplinary team meetings.
  • Provide support with activities of daily living (ADLs) and promote residents' comfort and dignity.
  • Maintain accurate and timely documentation in medical records and electronic health systems.
  • Ensure compliance with infection control policies and safety standards.
  • Supervise and direct Certified Nursing Assistants (CNAs) as needed.
  • Respond to emergency situations promptly and appropriately

Knowledge, Skills and Abilities

  • Strong clinical and communication skills.
  • Ability to work effectively in a team environment.
  • Familiarity with electronic medical records (EMR) systems a plus

Education & Experience

  • Current and valid LVN license in [State].
  • CPR/BLS certification (required).
  • Prior experience in a skilled nursing or long-term care setting preferred
