Essential Duties and Responsibilities
- Provide exceptional psychosocial support to hospice patients and their families.
- Conduct thorough assessments of patients' emotional, social, and psychological needs, and develop comprehensive care plans accordingly.
- Collaborate with the interdisciplinary team to ensure coordinated and holistic care.
- Offer counseling and emotional support to patients and their families, helping them navigate the challenges and emotional complexities of end-of-life care.
- Connect patients and families with community resources, support groups, and other relevant services.
- Advocate for patients' rights and empower them to make informed decisions regarding their care.
- Maintain accurate and confidential documentation of all interventions and assessments.
- Provide bereavement support to families during the grieving process.
Knowledge, Skills and Abilities
- Exceptional interpersonal and communication skills, with the ability to build trust and rapport quickly.
- Deep understanding of end-of-life issues, grief, and bereavement support.
- Strong assessment, problem-solving, and decision-making skills.
- Ability to work independently and as part of a team, demonstrating flexibility and adaptability.
- Proficiency in electronic medical records systems and documentation.
Education & Experience
- Bachelor's or Master's degree in Social Work from an accredited institution.
- Minimum of 2 years of experience in a healthcare or hospice setting (experience in hospice preferred).
