Essential Duties and Responsibilities

• Perform in-home mobility and functional assessments.
• Develop and execute individualized rehabilitation plans.
• Provide hands-on therapy, strengthening exercises, and mobility training.
• Recommend home safety modifications and adaptive equipment.
• Communicate with nurses, caregivers, and the care team to ensure coordinated care.

 Knowledge, Skills and Abilities

• Passion for individualized, patient-centered therapy.

 Education & Experience

• Active Minnesota Physical Therapist license.
• Valid driver's license and reliable transportation.
• Home health experience preferred, but outpatient and inpatient PTs strongly encouraged to apply.
