Essential Duties and Responsibilities

  • Carry out individualized OT treatment plans prepared by licensed Occupational Therapists.
  • Lead therapeutic activities focused on ADLs, fine motor development, motor planning, and cognitive engagement.
  • Educate clients and families on home safety strategies, adaptive tools, and functional routines.
  • Work closely with PTs, OTs, nurses, and caregivers to ensure continuity of care.
  • Document visits efficiently and accurately to support excellent outcomes.

 Knowledge, Skills and Abilities

  • Reliable, insured vehicle and valid driver's license
  • Strong communication skills and a heart for patient-centered care

 Education & Experience

  • Active Minnesota Certified Occupational Therapy Assistant (COTA) license
  • Home care experience is a bonus—training provided for eager learners
