Essential Duties and Responsibilities
- Provide individualized speech, language, and swallowing therapy in clients' homes.
- Conduct diagnostic assessments and create tailored treatment plans.
- Offer functional communication and cognitive-linguistic training.
- Educate clients and caregivers on strategies that reinforce progress and safety.
- Document services using EMR and communicate with the care team as needed.
Knowledge, Skills and Abilities
- Passion for patient-centered, one-on-one home care.
Education & Experience
- Current Minnesota Speech Language Pathologist (SLP) license.
- Valid driver's license and reliable transportation.
- Experience with EMR preferred.
